Primary Care - Medical Director (PACE Program)
Location: Cincinnati, OH
Setting: PACE (Program of All-Inclusive Care for the Elderly)
Employment Type: Full-Time, permanent position
Position Highlights:
* Blended leadership and clinical role within a high-volume, interdisciplinary care model
* Provide care for frail, low-income seniors dually eligible for Medicare and Medicaid
* Lead a collaborative team ensuring right care, right setting, right time
* Drive outcomes through evidence-based primary care and innovative care coordination
* Oversight of clinical quality, utilization management, and provider performance
* Strategic input into program development and operations
Leadership Responsibilities:
* Oversee all primary care and clinical activities of the PACE program
* Serve as a key member of the PACE leadership team
* Shape and support a robust interdisciplinary team model and culture
* Direct quality improvement and clinical outcomes initiatives
* Manage relationships with external hospitals, SNFs, and specialists
* Supervise and mentor physicians and advanced practice providers
* Participate in strategic planning, board meetings, and executive initiatives
Clinical Overview:
* Full-time schedule (40 50 hours/week) with shared after-hours call
* Several daily office visits with some home and hospital visits
* Panel size: 60 100 medically complex, nursing-home eligible seniors
* Participant-centered model addressing both medical and social needs
Ideal Candidate:
* MD or DO licensed in Ohio
* Board-certified in Internal Medicine or Family Medicine
* Additional certification or training in Geriatrics preferred
* Palliative care/hospice experience preferred
* At least 3 years of healthcare leadership experience
* At least 1 year of experience with frail, elderly, or long-term care populations
* Proven skills in chronic disease management, physical assessment, and team-based care
* Experience in team development, conflict resolution, and care coordination
* BLS certified and medically cleared for communicable diseases
