Position Details:
Patient Population: Geriatric patients in nursing facilities and advanced care coordination programs
Setting: Nursing facilities and outpatient geriatric care
Responsibilities:

Provide geriatric care across eight local nursing facilities

Support the Trinity Health at Home Thrive Program

Teaching opportunities with Residents, Medical Students, and Fellows

Shared call among providers
Required Procedures: Geriatric assessments and management of complex chronic conditions
EMR System: Not specified
Support Staff: Multidisciplinary geriatric care team
Days Per Month: Full-time (Monday–Friday)
On-Call/Weekends: Call shared among providers; flexibility in schedule
Submission Requirements:

Board Eligible (must become Board Certified within 2 years) OR Board Certified in Geriatric Medicine, Internal Medicine, or Family Medicine with geriatrics experience – Required

Experience working with geriatric population – Required

MI license (or willing to obtain) – Required

DEA (or willing to obtain) – Required

BLS and ACLS (or willing to obtain) – Required
