Job Responsibilities

The Geriatric Physician, under the supervision of the Medical Director, plays a crucial role in providing comprehensive medical coverage and care coordination for participants within an interdisciplinary team (IDT) environment. Essential functions include delivering complex primary care and acute illness treatment through in-person and telehealth visits, conducting comprehensive assessments, and actively participating in the IDT. The role involves collaboration with case management RNs, contributing clinical input in Transitions of Care settings, and conducting post-hospital and post-ED discharge visits. Additionally, the Geriatric Physician supervises and supports Nurse Practitioners and Physician Assistants, serves as a liaison between outpatient and inpatient specialists, and conducts visits to assisted living facilities and skilled nursing facilities based on clinical criteria. The position also involves reviewing outcomes, coding, and utilization data to enhance participant health and reduce unnecessary utilization. Rotation in night and weekend call duties via telephone is required.

Qualifications

• Completion of a residency program accredited by ACGM
• MD or DO degree
• Board Certification – Internal Medicine or Family Practice
• Additional certifications in Geriatrics, Hospice and Palliative Care, Addiction Medicine or Pain is desirable
• Active CA physician license without restrictions
• Current Basic Life Support (BLS) training
• State CDS Certification and DEA Registration
