Job Summary

CHI Saint Joseph Medical Group is searching for a geriatric medicine physician to help us transform healthcare for seniors. Partnering with our own clinically integrated network (ACO) we need a detail oriented driven physician to help us provide world-class primary care and coordinated care to the most vulnerable population, seniors. Our goal is to reduce doctor-to-patient ratios, and increase patients’ “face time” during each monthly appointment to help enhance doctor-patient relationships thus increasing patient compliance with treatment plans and decrease hospital admissions.

Essential Duties:

  • Understand, support, and implement the mission of the facility to deliver health care to those in need in the performance of all job responsibilities.
  • Ensure that patient care meets or exceeds the facility quality standards.
  • Ensure that patient satisfaction meets or exceeds standards set by the facility.
  • Educate patients on both clinical and managed care.
  • Participate in clinic and network task forces as requested and appropriate.
  • Function as an effective clinic team member.
  • Meet patient outcome standards
  • Effectively use clinic financial and other performance information.

Education:

  • Board Certified - or eligible.

License/Certification:

  • Kentucky Licensure.
