Locum doc needed to start December 11th to see geriatric patients in the clinic and SNF. Preventative visits, acute needs, and chronic ongoing needs. SNF admissions, discharges, acute needs, and periodic assessments. 1.5-2 days in the clinic and 3-3.5 days in the skilled Nursing Home.
8-12 patients per day at SNF and up to 16 patients in 8 hrs in clinic
SNF uses PCC and EPIC/ clinic is EPIC only
Clinic patients over 65 are seen for acute issues, chronic conditions, and physical and Medicare annual wellness visits. No procedures for MA support and RN triage support
SNF patients are admitted, discharged, and acute issues. The facility provides support for patient care. We have other MDs and NPs that make up the team to support patient care. No procedures
