Practice Highlights

  • FM/IM-to provide primary care services to SR Care clients at participating LTC facilities.
  • Care is provided in contracted SNFs, AFHs, and ALFs in our area.
  • Work collaboratively with the SR Care Clinical Director, Program Manager, and other staff to meet the program needs of the patients and facilities.
  • Integration and management of care to include inpatient admitting, clinical, and administrative services.
  • Strong clinical and elderly care with preference for an internal medicine background or geriatrician.
  • Bilingual in Spanish would be a plus.
  • EMR: GEHRIMED

  • Annual Base Salary: $200k-$210k includes fellowship training (if applicable), and experience. Additional productivity incentives.

Qualifications

  • MD/DO degree or IMG equivalent.
  • Completion of an accredited US FM or IM Residency.
  • Preference of geriatric medicine certification, or FM/IM with added qualifications in geriatrics, and/or AMDA cert or training.
  • WA License or the ability to obtain a WA license.
  • DEA certificate.
  • BE/BC required.
  • Great peer references and in good standing.
  • Approved for credentialing with Medicare, Medicaid, and health plans.
