Home Visit/Virtual Health position in South Bay, CA, Physician-led organization with team care model! JOB ID 68482

Job Details:

• BC in FM, IM or Geriatric (or have an interest in Geriatrics)
• At least two years of clinical experience with home-based care and/or Geriatric Medicine is strongly preferred
• Primary Care for Seniors, Majority home-bound
• 6-8 patients per day (For Follow up Visits, Physician decides if it will be virtual or in person depending on the patient)
• Patients' homes within driving distance of the Physician
• They have a mobile lab and xray when patients can't go to a nearby facility
• Provide laptops or tablets EMR is Athena
• Reimburse the federal rate for mileage
• Care Team Model
• 80% patient care (mostly home visitation, some vitual) and 20% admin (done from home)
• On call nights and weekends by phone 1 week per month as well as back up weeks
• Quality over quantity
• Value-based care physicians have an entire team dedicated to the Doctors, the patient and family
