- Demographics 16 years of age and older
- Patient Volume - 12-16 per day
- Case logs, documentation of provision of endocrinology, diabetes and metabolism services representative of the scope and complexity of the privileges requested ruing the previous 24months
- Support Staff- Yes
- Willing to license?- No provider must have active WA license, IMLC also would not be considered at this time due to urgency
- Certs- BLS
- EMR- EPIC
Schedule Details
- Schedule /Shift hours: M-F 8a-5p (8hr)
- Start - ASAP (End of February with request for urgent credentialing) License must be in hand at time of presentation
- CALL REQUIREMENT: Answering service pager services only during business hours 8a-5p for hospital patients only
