DURATION- 6 months

No Rounding/Admissions
Demographics 16 years of age and older
Patient Volume - 12-16 per day
Case logs, documentation of provision of endocrinology, diabetes and metabolism services representative of the scope and complexity of the privileges requested ruing the previous 24months
Support Staff- Yes
Willing to license?- No provider must have active WA license, IMLC also would not be considered at this time due to urgency
Certs- BLS
EMR- EPIC
