ThedaCare, located in Northeastern Wisconsin is seeking an Endocrinologist to join our provider group at ThedaCare Regional Medical Center - Neenah.
  • Full-time or part-time opportunities
  • Team consists of 3 physicians, 2 APC's, RN's, MA's and administrative support
  • No call
  • 100% outpatient clinic model with a high volume of patients
  • Patient population: 60% diabetes management, 40% thyroid/endocrine
ThedaCare's Endocrinology Service Line is building a Diabetes Center of Excellence. Our comprehensive diabetic care model is grounded in six pillars that serve as our guiding principles for developing expertise in diabetes care and delivering exceptional outcomes for diabetic patient population.

1. Team-based care with a focus on high-risk individuals
2. Coordination of care and communication across our medical community to help guide care
3. Comprehensive care including Diabetes and Endocrinology subspecialties
4. Culture of continuous learning and quality improvement
5. Evidence-based care and education on best practices
6. Diabetic population health and outcome assessment

Our Diabetic Care Team provides diabetes self-management education to new and existing patients, and instructions for the administration of injectable medications; utilizes leading diabetes-related technologies (e.g., meters, insulin pumps, continuous glucose monitoring); downloads and analyzes glycemic data at the point of care and between visits to guide timely modification of therapy when indicated; discusses diet, exercise, and medication use in relation to patient lifestyle; addresses management -related phone calls, e-mails, and texts in a timely manner; provides effective professional mental health and social support, including identification and access to community resources; provides professional education to ThedaCare providers and across our service area; educates family members on how to help with improving outcomes; and sorts out insurance issues for medication pre-authorizations according to tiers of coverage.

ThedaCare is a population-health-focused organization with over 1,300 providers across all specialties providing clinically integrated care as part of our ThedaCare ACO. Our Endocrinology Service Line collaborates with over 200 primary care providers in the management of our diabetic patients and endocrinological disorders.

The Fox Cities of Wisconsin includes 19 communities along the Fox River in East Central Wisconsin, including Appleton, Neenah, Menasha, Kimberly and a number of additional villages and townships. Qualifications

Will obtain Wisconsin Medical Licensure
Board Certified or Board Eligible (if board eligible must obtain certification within 5 years of employment)
