**If provider has a clean file, they may be able to cover December dates. Physicians with large settlements, DUIs/drug issues, restrictions, terminations, Medical Board actions, etc. are not eligible for temporary privileges.** If not, they will start in January.

CREDENTIALING

• Boards: BC or BE within 3-5 years of residency

Attached is a new privileging form that will set expectations for what is needed once a provider is confirmed.

Please note the following travel deviations from our contract will apply to the assignment (or current confirmation agreement):

• NOT reimbursed:

o Mileage from provider’s home to departing airport, as well as from airport to home upon return.
o Ride share (Lyft/Uber/etc.) and/or taxi from provider’s home to departing airport, as well as from airport to home upon return.

• WILL be reimbursed:

o Airport parking at provider’s departing airport if provider drives themselves to the airport.
o Insurance policies for rental cars must be pre-approved, prior to booking, otherwise it will not be reimbursed.
o Ride share (Lyft/Uber/etc.) and/or taxi from destination airport to hotel/lodging for assignment, as well as from lodging to hospital/clinic, and return tr
