July 8th: 9pm-6am
July 12th: 9pm-6am
July 26th: 11pm-8am
July 28th: 11pm-8am
July 31st: 11pm-8am

*Update 6/22* All providers presented should be presented with 2-3 reference options for a reference check by the medical director. Please ensure this is included on the bid. MD's name, email and phone number. These can be uploaded after the bid is sent in.

*WE CAN ONLY LOOK AT PROVIDERS WITH CLEAN FILES* If you present at provider with malpractice or an NPDB that is not clear, we will not name clear the candidate and decline. What we need:

• A "clean" applicant

o Physicians with large settlements, DUIs/drug issues, restrictions, terminations, Medical Board actions, etc. are not eligible for temporary privileges

• The RFA, CV, and 24 months of relevant activity
• A completed application separate from / following the above step
• References on standby to respond immediately

JOB DETAILS:

Average number of patients in 24hrs is 110.

Please ensure the bid reflects the below:

• Guaranteed Hours: 8
• OT applies after: 12

Although no OT is expected and should be pre-approved by the medical director.

REQUIREMENTS:

Candidate MUST have an ACTIVE CA license.
Must be BC or BE within 5 years.
ABEM/ABEOM certified (Will not accept ABIM/ABFM)

Please note the following travel deviations from our contract will apply to the assignment (or current confirmation agreement):

• NOT reimbursed:

o Mileage from provider’s home to departing airport, as well as from airport to home upon return.
o Ride share (Lyft/Uber/etc.) and/or taxi from provider’s home to departing airport, as well as from airport to home upon return.

• WILL be reimbursed:

o Airport parking at provider’s departing airport if provider drives themselves to the airport.
o Insurance policies for rental cars must be pre-approved, prior to booking, otherwise it will not be reimbursed.
o Ride share (Lyft/Uber/etc.) and/or taxi from destination airport to hotel/lodging for assignment, as well as from lodging to hospital/clinic, and return tr
