Modality: Emergency Medicine Physician

(Will take EM or IM/FM with ER experience)
Openings:

Hamilton, NY
Shifts: Provider needs to be able to commit to at least 7 shifts per month and be open to days and nights
7am-7pm
7pm-7am
Shift Days: Sun-Sat
mainly days, but must be willing to do nights (do NOT submit anyone that only wants days)
Hours per Week: N/A
Contract Length: June 2026 start date target but will start/orientation once privileges are received

Ongoing contract
Details of Assignment: - no trauma level designation
- no scribe coverage
- annual patient volume: 13K (lower to low-med volume)
- patients per day: 35%
EMS: 6.40%
admitted patients: 6.20%
- 24 hour MD/DO physician coverage
- travel/lodging fully covered by client
Charting System: EPIC
Client Interview Required: no



Minimum Experience and Certification Required: - Must be board-certified in EM, FM, or IM or Board eligible with requirement to become board-certified within 5 years of residency completion
- must have NY DEA in-hand at time of submission - must included Schedules 2, 2N, 3, 3N, 4, and 5
- if FM/IM, need to have ACLS, PALS (through AHA), and ATLS
- if EM trained, must have ACLS (through AHA) and PALS (through AHA)
- Must have NY license in hand at time of submission
- if IM/FM, must have at least 2 years of EM exp and some of that has to be within the last 2 years


Facility Details

Orientation: Typically a few hours in one day and then can shadow another physician. Can likely do block of working shifts in the subsequent days following orientation

Temps can be issued after the following are received: 3 refs, NPDB, bg ck, verification of ed, training, boards and affiliations

Onboarding requirements (orientation/EMR training): PCC sets up orientation (ss, dl, current ppd, current flu, diploma, residency cert, board cert, NY med license registration, NY DEA, NY infection control cert, photo, CMEs from past 2 years, case logs from past 2 years, ACLS, PALS, ATLS

$50 app fee for physicians, COIs from past 5 years

Immunization & Health Requirements:
Annual flu vaccine.
PPD or Chest X-ray at initial credentialing.
Initial physical exam.
Titers for MMR, Hepatitis B, and Varicella.
N95 annual fit test.

Case Logs: Must provide case logs covering the past 2 years.
