| Details of Assignment: | - no trauma level designation - no scribe coverage - annual patient volume: 13K (lower to low-med volume) - patients per day: 35% EMS: 6.40% admitted patients: 6.20% - 24 hour MD/DO physician coverage - travel/lodging fully covered by client |

| Minimum Experience and Certification Required: | - Must be board-certified in EM, FM, or IM or Board eligible with requirement to become board-certified within 5 years of residency completion - must have NY DEA in-hand at time of submission - must included Schedules 2, 2N, 3, 3N, 4, and 5 - if FM/IM, need to have ACLS, PALS (through AHA), and ATLS - if EM trained, must have ACLS (through AHA) and PALS (through AHA) - Must have NY license in hand at time of submission - if IM/FM, must have at least 2 years of EM exp and some of that has to be within the last 2 years |
