- Board Certified/Board Eligible: Board Certified in EM/FM/IM, or Board Eligible with requirement to become Board Certified within 5 years of residency completion
- Licensure: Active, unrestricted New York Medical License. Active DEA registration with a New York address; must include Schedules 2, 2N, 3, 3N, 4, and 5
