- Board Certified
- Active MS License or IMLC
- Active MS DEA
- Maintenance of Board Certification required for reappointment eligibility
- Successful completion of an ACGME or AOA-approved residency training program
- Residents in year 2 or greater are eligible for Emergency Medicine Privileges
- Applicants must demonstrate active practice in the past 12 months
