- Licenses: Active LA license or IMLC eligible; CDS; DEA (with LA address)
- Board: BC/BE in EM, FM, or IM (must have EM experience)
- Certifications: ACLS, ATLS, PALS (required unless ABEM certified & participating in ABEM continuing certification)
- Residency Training: EM, FM, or IM
- Experience: Minimum 2 years ED experience (required for FM/IM trained)
