Requirements:
• Board Certifications Req’d/Preferred: BC in Family Practice
• State License(s) / IMLC Req’d?: KY license required
• Certifications Req’d?: ACLS
• DEA Req’d?: Yes
• Minimum Experience Req’d: 2 years minimum experience in ER if ABFM provider
• Board Certifications Req’d/Preferred: BC in Family Practice
