- Facility volume: 25K
Requirements
- Active TX License
- Active TX DEA
- Board Certified in ABEM OR ABIM (or osteopathic counterparts)
- Must be residency trained
- ACLS, PALS
- Must have ATLS unless Emergency Medicine Residency trained or Board certified in Emergency Medicine
- CME past 2 years, case logs past 2 years, copies of all education & training diplomas, SS Card, Driver's license, color photo TB/FLU/Hep B/MMR/VARICELLA Immunization records
