Setting: Federally Qualified Health Center (FQHC) - Multiple Locations
Position Overview
A mission-driven community health organization is seeking a dedicated Part-Time Cardiologist to provide high-quality cardiovascular care to adult patients in an outpatient, community-based setting. This role focuses on improving access to specialty cardiac care while working collaboratively within an interdisciplinary care model to enhance patient outcomes.
Key Responsibilities

Provide comprehensive outpatient cardiology consultations and follow-up care

Evaluate, diagnose, and manage acute and chronic cardiovascular conditions

Interpret cardiovascular diagnostic tests as appropriate

Develop individualized, evidence-based treatment plans

Collaborate closely with primary care providers, nursing staff, and care coordination teams

Ensure timely referrals, continuity of care, and accurate clinical documentation

Participate in quality improvement and patient care initiatives

Maintain compliance with federal, state, HRSA, and managed care guidelines

Deliver patient-centered and culturally responsive care to diverse populations
Qualifications

MD or DO from an accredited medical school

Board Certified or Board Eligible in Cardiology

Active and unrestricted New York State medical license and DEA (or ability to obtain)

Hospital admitting privileges or ability to obtain

Strong commitment to community-based and patient-centered care
Preferred Qualifications

Experience working in an FQHC, community health center, or safety-net setting

Experience serving diverse and underserved populations

Bilingual skills (Spanish or other languages) are a plus
