Job description
Our value-based care organization, Conviva Senior Primary Care, is seeking a non-invasive cardiologist to lead the development of a new outpatient cardiology program in Jacksonville, Florida.
The Cardiologist will be part of our value-based clinical care team that focuses on outpatient medicine, continuity of care, health maintenance, and disease prevention for elderly patients. The Cardiologist manages complex cardiac conditions such as heart attacks, abnormal heartbeat rhythms, and congestive heart failure, as well as provides preventative techniques for dealing with potential heart-related illnesses. The Cardiologist spends 100% of their time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, etc.) as it relates to direct patient care.
- Guaranteed base salary + 20% quality incentive bonus, total compensation in the high $500,000's

Job Responsibilities
Qualifications
• Current and unrestricted medical license or willing to obtain a medical licenses in state of practice;
• Graduate of accredited MD or DO program of accredited university
• Excellent verbal and written communication skills
• Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients
• Fully engaged in the concept of “Integrated team based care” model
• Willingness and ability to learn/adapt to practice in a value-based care setting
• Willingness to drive between two to four centers in personal vehicle; Conviva offers mileage reimbursement
Preferred Qualifications:
• Board Certification or Eligible to become certified (ABMS or AOA) in Cardiovascular Disease
• Board Certification in echocardiography, nuclear cardiology, and vascular ultrasound (RPVI) preferred
