Coverage Needed:
- Start Date: As soon as credentialed
- End Date: 6 months from start date
- Schedule: Inpatient & Outpatient; 1 day clinic
- Call: 1:4
Clinical Details:
- Required Procedures: Standard interventional cardiology core procedures
- EMR: EPIC (experience preferred, but training available)
- Support Staff: Medical Assistants and Registered Nurses
- Patient Volume: 10-25 in clinic; 10-50 inpatient
- Assist with both interventional and non invasive
Requirements:
- Will be required to work at 2 sites, one in Indiana, and one in Illinois
- Will need active Medical and Controlled Substance Licenses in BOTH Indiana and Illinois at the time of submission
- Board Certified
- CV must be in Month / Year format
- Must have license in hand at time of submission, or proof that the license application has been submitted
- Any gaps greater than 30 days will require an explanation on the CV
- DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
