JOB OVERVIEW
• Job Title: Physician – Electrophysiologist
• Job Type: Locum Tenens
• Location: Flagstaff, AZ
• Service Setting: Inpatient/outpatient (In-office clinic)
• Coverage Type: Clinical Only
• Coverage Period: December 29, 2025 – Ongoing
• Clinical Shift Schedule: Mon–Fri, 7:00 AM – 5:00 PM
COVERAGE DATES
• December 29, 2025 – January 2, 2026
• Plus 1–2 weeks per month in 2026
PATIENT INFORMATION
• Patient Demographics: Adults
• Patient Volume/Census:20-22 patients per day
• Consults: 3–4 per day
• EP Cases: 4–6 per day, depending on case length
• Admissions: Yes
• Case Mix: Cardiac electrophysiology; heart rhythm disturbances, left-sided ablations
FACILITY INFORMATION
• No. of Beds in Department: 268
• EMR System: Cerner
• Reason for Coverage: Supplemental
PRIVILEGES & COMPLIANCE
• Hospital Privileges Required: Yes
• Credentialing Timeline: 30 Days
COMPENSATION & BENEFITS

• Travel, Lodging, and Malpractice Insurance: Covered
REQUIRED PROCEDURES
• Left-sided ablations
• Cardiac electrophysiology diagnostics and procedures
JOB REQUIREMENTS
• Licenses: Active Arizona license or IMLC; willing to license
• Board: BC/BE in Electrophysiology
• DEA: Active DEA with AZ address
• Certifications: ACLS required
• Experience: Minimum 1 year in Electrophysiology
DUTIES & RESPONSIBILITIES
• Provide high-quality inpatient and outpatient cardiac electrophysiology care
• Perform and interpret specialized EP procedures
• Consult on 3–4 hospital cases per day and conduct 4–6 EP procedures daily
• Collaborate with Cath Lab team and other cardiac specialists
• Document encounters using Cerner EMR system
