Qualifications
License required: AZ Licensed, will look at IMLC
DEA: AZ registered
CSR: Not required
Certifications required: ACLS
Minimum experience required: 1 year
Credentialing Timeframe: 30 days

Shift Details:
Monday – Friday
Two weeks per month
7:00am-5:30pm – Cath Lab at the Hospital
EXCEPT FOR THE FOLLOWING DAYS
Every Wednesday 8:00am -5:00pm – In- Office Clinic
Every 2nd Friday of the month 8:00am -5:00pm – Telemedicine Clinic
Every 4th Friday of the month 8:00am -5:00pm – In-Office Clinic (First Patient at 9:00am)
