MD:CRNA Ratio: 1:3–4

Case Mix: Ortho, Urology, Eyes, Endo

Patient Population: Adults and limited Pediatrics (Dental/Eyes only)

Supervision: Mix of own cases and CRNA supervision

✅ Submission Requirements

Board Certified – REQUIRED
-
Active New York State License at time of submission – REQUIRED

Active BLS, ACLS, PALS – REQUIRED

Providers must be outside a 60-mile radius – REQUIRED

Must be available to work at least two weeks per month – REQUIRED

Full-time availability – PREFERRED
-
Self-Query NPDB report (pulled within the same month as submission) – REQUIRED
