- Dates of Coverage: Full time to start ASAP
- Reason for Coverage: open slot
- Practice Setting: Hospital Based
- Will the Locum Tenen Physician be Required to Supervise CRNAs?: both
- Patient Age Demographic: Geriatric
- Number of Operating Rooms: 8
- What is the ratio of own cases to CRNA supervision? Mix daily
- EMR: Epic
- MD:CRNA Ratio? 1:3/4
- Breakdown of case mix? Ortho/ urology/ eyes/ endo
- Patient demographic: Adults/ peds: dental eyes only
- Facility will reimburse for the following costs: Flight, Hotel, Rental Car

Submission Requirements
- Board Certified - [REQUIRED]
- Active NY State License at time of submission - [REQUIRED]
- Active BLS, ACLS, PALS - [REQUIRED]
