Procedural Skills Required: Epidurals, Regional Blocks, Spinals (case logs required as proof)

Requirements

Board Certification: Required (will not accept BE)

State License: Indiana – active at time of submittal

Certifications: ACLS, BLS, PALS

Vaccinations: COVID-19 required (exemptions accepted)

Experience: Case logs must show proof of spinals, blocks, and epidurals

Credentialing Timeframe: ~120 days
