Requirements

Board Certification: Required – Anesthesiology

License: Indiana (active at time of submittal)

Certifications: ACLS, BLS, PALS (all required)

Case Logs: Must show proficiency with epidurals, spinals, and blocks

Credentialing Timeframe: ~120 days
