Practice Setting: OR/Surgery Department
Dates/Coverage Needed: Beginning ASAC - FT/ongoing for 3-6 months with the option to extend. Provider must be willing to take calls and share them equally with all staff.
Case Mix: All case types are listed, excluding hearts, heads, and peds.
Credentialing Time-Frame: 90 days
Active AZ License: Yes
Certifications: ACLS, PALS, and they must be AHA or American Red Cross
Flight/Lodging/Rental Car or Mileage: Yes
