LOCATION DETAILS: 1 hour 10 north of Springfield, IL

Case Mix: Bread and Butter, Ortho, GI, Endo, etc … (peds not required for locums)(If provider would like to do peds, they can… cardiac peds ages 1 and 2 and up. OB, Heads, hearts not required
· Required procedures include: none, must just be able to do bread and butter. OB not required

Qualifications
o Licensing: OPEN TO PROVIDERS WHO NEED TO GET IL license and IL DEA, controlled substance
o Minimum Years of Experience? No new grads, 2 years of experience minimum
o Board Status: BE/BC within 5 years of completing training
o COVID Vaccination Required? No
o Certifications/Cred Items: ACLS, BLS
