Primary Purpose
Responsible for providing quality management of patients for a variety of medical and surgical conditions, and for developing, implementing and monitoring quality standards of patient care to ensure optimal patient care, coordinated patient services and cost effectiveness in the Geriatric specialty area.

Minimum Specifications


Education

  • Must have successfully completed an Accreditation Council for Graduate Medical Education (ACGME) or American Medical Association (AMA) approved residency for appropriate specialty.

Experience

  • None.

Equivalent Education and/or Experience

Certification/Registration/Licensure

  • Must be currently licensed for the practice of medicine in the State of Texas.
  • Must meet and maintain licensing and registration requirements as delineated by the Medical Staff Bylaws, Medical Staff Rules and Regulations, and Credentialing Policies/Procedures, if any.

  • Must be able to demonstrate a working knowledge of the appropriate specialty practice (e.g. Psychiatry, Family Practice, Internal Medicine, Pediatrics, Jail Health) assessment techniques and treatment processes.
  • Must be able to determine objectives, set priorities, and establish and meet target dates.
  • Must be able to work effectively with other health care professionals and serve as a resource to the community.
  • Must be able to demonstrate patient centered/patient valued behavior.
  • Preferred: Spanish proficiency is preferred

Responsibilities

  1. Patient Care
  2. Gathers and documents essential and accurate information about patients; makes informed decisions about diagnostic and therapeutic interventions incorporating patient information and preferences; develops and carries out patient management plans on an ongoing basis.
  3. Provides health care services in the following categories of care including age appropriate health maintenance and immunization, early diagnosis and treatment for those with acute illness, planned age appropriate evidence-based management of those with acute and chronic conditions, and focus on improvement of quality of life.
  4. Counsels and educates patients and their families regarding health promotion, appropriate management of acute conditions, and self-management of chronic conditions.
  5. Collaborates with other health care professionals within the health center to provide patient-focused care, to deliver care efficiently and effectively, and to meet patient needs.
  6. Coordinates care with appropriate health care professionals at PHHS or within the community including specialty care and allied team members.
  7. Interpersonal and Communication Skills
  8. Utilizes interpersonal and communication skills that result in effective information exchange and partnership with patients and families. Creates and maintains a therapeutic and ethically sound relationship with the patient. Demonstrates respect, compassion, integrity and responsiveness to the needs of the patient.
  9. Utilizes interpersonal and communication skills to work effectively with colleagues within the care team, the health center, and across the PHHS system.
  10. Practice-Based Learning and Improvement
  11. Maintains knowledge and works toward the division quality goals to achieve the desired practice outcomes.
  12. Identifies ways to improve work processes, workflows and customer satisfaction for assigned area. Modifies approach, communication and interaction to take into account the specific or special needs of all customers. Recommends appropriate changes to improve effectiveness, productivity, and efficiency that support the overall goals of the department and PHHS.
  13. Demonstrates a commitment to personal professional development. Stays abreast of the latest developments, advancements, and trends in the field of medicine by attending seminars/workshops, reading professional journals, actively participating in professional organizations. May participate in writing and/or presenting publications and presentations. Integrates knowledge gained into current work practices.
  14. Maintains knowledge of applicable rules, regulations, policies, laws and guidelines that impact the field of medicine. Develops effective internal controls to promote adherence to applicable state/federal laws, and the program requirements of accreditation agencies and federal, state, and private health plans.
  15. System-Based Practice
  16. Practices cost-effective health care. Ensures clinical visit expectations and access standards established by the program or health center are met.
  17. Advocates for quality patient care within the system and community to maintain quality standards and assist patient in dealing with system complexities to include acting as a patient advocate within the system.
  18. Utilizes established clinical practice guidelines and referral protocols to effectively manage care within the system. Understands how their patient care and other professional practices impact other healthcare professionals within the organization.

Job Accountabilities

Develops effective internal controls designed to promote adherence with applicable laws, accreditation agency requirements, and federal, state, and private health plans. Seeks advice and guidance as needed to ensure proper understanding.
