DATES:
As soon as credentialed - ongoing
Three days per week
Flexible on what days the provider can coverage (not weekends)
SHIFTS:
6 am - 11 am
Outpatient clinic
15 - 20 patients per day
REQUIREMENTS: Board Certified, local Wisconsin physician, active Wisconsin license, Suboxone waiver not required
