As a full time Behavioral Health Consultant based in Sound Bend, IN, your role will involve advocating for and assisting assigned patients in accessing vital resources to address their medical and behavioral concerns. You will offer personalized recommendations and implement behavioral interventions to support patients on their journey to wellness. Collaborating closely with the Integrated Medical Team, you'll contribute to a comprehensive approach to healthcare. Reporting directly to the Chief Behavioral Health and Addictions Officer, you'll be an integral part of our mission to provide holistic care to our community.

Compensation: Competitive salary plus benefits

Requirements:

  • Masters degree in Social Work
  • LCSW, LMHC, (LMHCA only if approved for state LMHC exam) LMFT or LCAC
  • PH. D and APA certification (not required but highly preferred)
  • 1-2 years experience of post-degree clinical experience
  • Additional primary-care experience (not required but highly preferred)
  • Operate a multi-line phone system and other office equipment including printers, fax machines, etc.
  • Basic computer skills (Microsoft Office, EHR, online sources, etc.)
