Job Description
Our client is seeking Advanced Practitioners to join their team for geriatric primary care in southern Minnesota. Please see Job ID #64569
- Full-time, permanent position (exact schedule to be discussed during interview)
- Provider with be performing primary care duties going to assisted living facilities (travel required and compensated)
- Candidate is required to have at least 2 years of experience in a primary care setting, direct geriatric and assisted living experience preferred
- Duties include: Sick/well visits, physical examinations, vaccinations, and follow-up consults
- Must be able to treat chronic disease cases Hypertension, hyperthyroidism, diabetes, Arthritis, Asthma, etc.)
- Must be able to work autonomously
- Minnesota state license in hand or in process, Valid driver's license required
