Post-Acute Care | Tucson, Arizona | Full-Time
We are seeking a compassionate and driven Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing post-acute care team in the Tucson, Arizona area. This is an excellent opportunity for an experienced clinician who values autonomy, collaboration, and meaningful patient impact.
This full-time position (5 days per week) involves rounding in skilled nursing facilities, supported by a strong clinical and operational leadership team. We are specifically seeking a provider with at least two years of NP or PA experience, ideally with a background in Physical Medicine & Rehabilitation (PM&R) or physiatry.

- Perform comprehensive patient assessments, including medical histories and physical examinations
- Develop, implement, and adjust individualized care plans
- Monitor patient progress and coordinate care to optimize outcomes
- Collaborate closely with physicians, nursing staff, and interdisciplinary healthcare teams
- Educate patients and families on health management and preventive strategies
- Maintain accurate and timely medical documentation in compliance with regulatory standards
- Active Arizona NP or PA license
- Current DEA license
- Minimum 2 years of NP or PA experience preferred
- Experience in post-acute, acute care, emergency department, rehabilitation, or clinic settings preferred
- Strong clinical foundation and knowledge of healthcare regulatory standards
- Excellent communication and interpersonal skills
- Collaborative, team-oriented approach with a positive, patient-centered mindset
