Opportunity Overview
Full-time post-acute care opportunity for an experienced Nurse Practitioner or Physician Assistant providing on-site care in skilled nursing facilities in the Tucson area. This role offers strong clinical support, schedule flexibility, and uncapped earning potential.
Position Highlights
- Full-time: 4–5 days per week
- Rotating weekend coverage
- Day call on weekdays and weekends
- After-hours/night-time backup call only
- Coverage across 48–58 bed skilled nursing facilities
- Fee-for-service compensation model with uncapped productivity
Compensation & Benefits
- Estimated annual compensation: $143,000–$171,000 (uncapped productivity)

Clinical Responsibilities
- Conduct comprehensive patient assessments and follow-up visits
- Develop and manage individualized care plans
- Monitor patient progress and coordinate interdisciplinary care
- Collaborate with physicians, nursing staff, and rehabilitation teams
- Educate patients and families on treatment plans and recovery goals
- Maintain timely and accurate documentation in compliance with regulations
Candidate Requirements
- Nurse Practitioner or Physician Assistant with active Arizona license
- Minimum 2 years of clinical experience required
- Post-acute, rehab, PM&R, acute care, or clinic experience preferred
- Strong clinical judgment, communication, and teamwork skills
