Job Summary:

We are seeking a skilled and compassionate Nurse Practitioner or Physician Assistant to join our team at an inpatient rehabilitation setting in the Chicagoland region. The successful candidate will work in both acute and subacute facilities, providing comprehensive rehabilitation services to patients with various physical and functional impairments.

Key Responsibilities:

  • Conduct thorough assessments of patients to determine their functional abilities, limitations, and needs
  • Develop and implement individualized treatment plans based on evidence-based practices and patient goals, in collaboration with the physician and other healthcare professionals
  • Order and interpret diagnostic tests and procedures, and adjust treatment plans as necessary
  • Provide ongoing education and support to patients and their families, and ensure timely and appropriate communication with other healthcare professionals
  • Monitor and document patient progress, and maintain accurate and up-to-date patient records in accordance with regulatory requirements
  • Participate in interdisciplinary team meetings and contribute to the development of policies and procedures related to rehabilitation services
  • Collaborate with therapists, nurses, and other healthcare professionals to coordinate patient care and optimize outcomes

Qualifications:

  • Master's degree from an accredited Nurse Practitioner or Physician Assistant program
  • Active license to practice in the state of Illinois
  • Minimum of 2 years of experience in a rehabilitation setting, preferably in an inpatient acute or subacute facility
  • Strong clinical skills and knowledge of evidence-based rehabilitation practices
  • Excellent communication, interpersonal, and organizational skills
  • Ability to work independently as well as collaboratively as part of an interdisciplinary team
  • Flexibility to work a variety of shifts, including weekends and holidays as needed
