Pediatric Nurse Practitioner Opening – Portage, IN Located in Portage, Indiana - Chicago 40m. This full-time Pediatric Nurse Practitioner role provides primary care to children from birth through young adulthood, with a strong focus on prevention, well-child care, and management of common acute and chronic pediatric conditions within the context of family, community, and environment. About the Role -Full-time, exempt position -Primary care pediatrics: newborn through young adult -Focus on health maintenance, promotion, prevention, and disease management -Designed to support the specialized physiologic and psychological needs of pediatric patients -Office-based clinical setting with frequent patient/staff interaction -High-touch care coordination and timely documentation expectations -EMR reporting and charting via NextGen -Travel outside of NorthShore locations is rare, but driving may be required for off-site meetings, hospitals, programs, training, and conferences Key Responsibilities -Obtain relevant health and medical histories -Identify health risks and needs; update and record changes in health status -Order appropriate diagnostic testing -Develop treatment plans and plans of care -Prescribe appropriate pharmacological agents -Prescribe non-pharmacological interventions -Develop and deliver patient/family education plans -Make referrals to other health professionals and community agencies as needed -Schedule follow-up care per best-practice standards -Document patient outcomes to evaluate effectiveness of care plans -Reassess and modify care plans to achieve clinical goals -Promote patient health through education and shared decision-making -Support appropriate utilization of healthcare resources -Complete charting and task response in a timely manner to prevent delays in care and support coordination -Collaborate with other providers to deliver comprehensive care -Participate in periodic quality assurance review, including systematic review of records and treatment plans -Prepare reports and documentation as required -Attend meetings and required training sessions -Execute EMR reporting through NextGen and perform other assigned tasks within scope Required Qualifications -Valid RN license -Valid APN license -Valid CRS certification -Valid federal DEA license -Board certified as a Pediatric Nurse Practitioner -Graduate of an approved Master’s Nursing Program for Pediatric Nurse Practitioners that meets Indiana licensure requirements -Meets credentialing guidelines of the organization -Eligible to bill through Medicaid and Medicare with no history of fraud (OIG expectations) -In good standing with the National Practitioner Data Bank Skills and Competencies -Planning and strategic foresight -Responsible decision-making -Integrity and accountability -Innovation and creativity -Adaptable and flexible approach -Leadership, teamwork, and conflict resolution -Professionalism and strong work ethic -Empathy and patient-centered communication -Ability to relate to individuals of diverse ages, ethnicities, cultures, socioeconomic backgrounds -Strong clinical judgment and care coordination capability -Proficiency with Microsoft Office Suite or similar tools -Comfort with clinical diagnostic equipment within scope -Comfort with standard office equipment and systems: computer, NextGen EHR, fax/copier/scanner, multi-line telephone Work Environment and Physical Requirements -Office environment with constant and interruptive interaction -Frequent personal and telephone contact with patients and healthcare personnel -Work can be stressful with frequent exposure to high-stress situations -Subject to exposure to bloodborne and airborne pathogens -Prolonged periods of walking, sitting, and standing -Frequent viewing of computer monitor -Frequent hearing and speaking -Frequent reaching with hands/arms -Frequent use of hands to touch, handle, grip, and feel -Ability to lift up to 50 lbs occasionally -Vision requirements include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus -Driving required for select off-site responsibilities
