Job description Lead Advanced Practice Provider – Nephrology & Hypertension Location: Jacksonville, FL Department: Nephrology & Hypertension Employment Type: Full-Time, permanent position Role Type: Clinical + Operational Leadership Position Overview This role combines advanced clinical practice with operational and people leadership. The Lead APP provides direct nephrology care while overseeing the performance, scheduling, development, and operational effectiveness of the NP/PA team across inpatient and outpatient settings, including dialysis services. Leadership Responsibilities * Serve as liaison between NP/PA staff, physician leadership, and administration * Ensure appropriate NP/PA staffing coverage for days, nights, weekends, holidays, and overnight shifts * Provide coaching, performance evaluations, and ongoing feedback * Lead staff meetings and communicate workflow and operational updates * Support onboarding and orientation of new APP team members * Identify and resolve operational and workflow issues * Contribute to policy development, process improvement, and operational planning Clinical Responsibilities * Provide direct patient care across the continuum of nephrology services * Manage patients with CKD, ESKD, AKI, hypertension, kidney stones, and acid-base/electrolyte disorders * Coordinate daily rounds with attending physicians and multidisciplinary teams * Perform comprehensive histories, physical exams, and diagnostic interpretation * Prescribe pharmacologic therapies and renal replacement therapies, including dialysis and CRRT * Monitor and assess effectiveness of therapeutic interventions * Deliver patient and family education on kidney disease and renal replacement options * Coordinate ESRD care with outpatient dialysis centers from admission through discharge * Participate in telehealth and video visits for inpatient and ambulatory care * Make appropriate specialty referrals and ensure continuity of care * Evaluate care plans for quality, outcomes, and cost-effectiveness Practice Setting * Hospital-based consulting service with 24/7 nephrology coverage * Minimum of 20% inpatient coverage, including possible nights and weekends * Occasional outpatient clinic responsibilities, including dialysis unit rounding * Collaborative environment with nephrologists, fellows, nurses, and allied health professionals Education & Academic Involvement * Precept and mentor APP fellows, residents, students, and new NP/PAs * Participate in educational lectures, grand rounds, quality initiatives, and faculty meetings * Support APP and physician fellowship training programs Qualifications * Nurse Practitioner or Physician Assistant with nephrology experience * Eligible to practice to full scope under Florida licensure * Strong leadership, communication, and organizational skills * Experience with inpatient nephrology and dialysis preferred * Demonstrated commitment to quality improvement and team development Compensation & Benefits * Competitive compensation package * Medical, dental, and vision insurance options * HSA and FSA pre-tax savings plans * Competitive retirement program * CME and professional development support * Comprehensive benefits eligibility
