Clinical Director leads a team of palliative care clinicians that care for a chronically-ill and vulnerable population, primarily in the patients’ homes. The Clinical Director is responsible for providing day-to-day management of all clinical staff and ensuring the market achieves all clinical performance goals including hospitalization reductions, goals of care discussions and home visit metrics. The Clinical Director organizes weekly interdisciplinary team meetings and call schedules and oversees on-boarding, orientation, ongoing trainings, performance evaluations, and mentorship of palliative care clinicians. Most importantly, the Clinical Director will set the cultural tone, establishing a highly professional, dedicated team of palliative care clinicians, caring for the area’s most vulnerable patients. The Clinical Director also will work with the Vice President of Operations to build a working relationship with health plan partners and provide education and outreach to primary care and specialist physicians regarding services.


A successful candidate will have previous supervisory or management experience (Required). Previous experience in home care, hospice, palliative care, or geriatrics strongly preferred; other complex patient health care experience also beneficial.


For Physician Assistant:
Graduate of a Physician Assistant program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) (Required)


For Nurse Practitioner:
Graduate of an approved Nurse Practitioner program (Required)

Licensure/Certification:
Must have or be eligible for appropriate APP licensure. CPR certification (Required). DEA possession or eligibility (Required). Must have reliable automobile, valid driver’s license and minimum state required liability auto insurance; APPs travel in personal vehicle with reimbursement for mileage
