Nurse Practitioner or Physician Assistant - Geriatric Primary Care


SCHEDULE:
- Full-Time
- 40 Hours per Week
- Monday to Friday
- No On-Call

COMPENSATION:
- $125,000 to $150,000+ Total Annual Income
- $60 to $65+ per Hour, negotiable based on experience

COMPANY PROFILE:
This compassionate non-profit is dedicated to delivering convenient, high-quality healthcare to patients in a variety of settings. They currently boast a team of 11 physicians, nurse practitioners, and physician assistants who, along with nursing, clinical, and administrative team members, ensure that patients receive the best medical care possible while working diligently to remove access barriers. Their services include home-based primary care, palliative care, hospice care, chronic illness management, home health, counseling services, and more.


POSITION DESCRIPTION:
The Nurse Practitioner or Physician Assistant is responsible for delivering comprehensive primary care to a dedicated panel of geriatric patients. This includes visiting assisted and independent living facilities (ALFs / ILFs), private homes (house calls), and other group settings (apartment buildings, senior housing complexes, etc.).

The duties of the role include, but are not limited to:

- conduct comprehensive physical examinations
- diagnose and treat illnesses
- manage chronic conditions
- engage in advanced care planning discussions
- coordinate care with other healthcare providers
- provide palliative care consultations

Providers see an average of six (6) patients per day. Based on patient need, more visits may be scheduled (if work hours permit) and are reimbursed in addition to the hourly rate.

The new provider will see medically complex, frail, and geriatric adults who may have:

- high clinical complexity
- multi-morbidity
- functional decline
- mobility limitations
- polypharmacy
- cognitive impairment
- caregiver burden
- increased risk for avoidable utilization
- evolving goals of care

This requires providers to build and maintain therapeutic relationships with patients and caregivers, including providing emotional support and education as needed.

The practice uses the AthenaCare EHR.



REQUIREMENTS:
- North Carolina NP or PA license
- DEA
- 1+ year advanced practice experience in a relevant environment
- previous home health or long-term care facility (as RN or APP) preferred
