SCHEDULE:
- Full-Time
- Monday to Friday
- 40 Hours per Week
- No Evenings

COMPENSATION:
- $60 to $65+ per Hour, negotiable based on experience

COMPANY PROFILE:
This compassionate non-profit organization is dedicated to delivering convenient, high-quality care to a variety of patients in a variety of settings. They currently boast a team of 10 physicians, nurse practitioners, and physician assistants who, along with dedicated nursing, clinical, and administrative team members, ensure that patients receive the best medical care possible while working diligently to remove access barriers. Their services include home-based primary care, palliative care, hospice care, chronic illness management, home health, counseling services, and more.


POSITION DESCRIPTION:
The Nurse Practitioner / Physician Assistant is responsible for delivering comprehensive primary care, palliative care, and chronic condition management to a dedicated panel of geriatric patients in place. This includes visiting private homes, apartment buildings, and long-term care settings such as assisted and independent living facilities.

The duties of the role include, but are not limited to:

- conducting physical examinations
- diagnosing and treating illnesses
- managing chronic conditions
- engaging in advanced care planning discussions
- coordinating care with other healthcare providers
- providing consult-only palliative care services as needed

The Nurse Practitioner / Physician Assistant provides care to medically complex, frail, and geriatric adults who may have one or more of the following situations: multi-morbidity, polypharmacy, cognitive impairment, functional decline, and significant caregiver burden.

Patients typically have mobility limitations and experience high clinical complexity, evolving goals of care, and increased risk for avoidable utilization. Providers are expected to build and maintain therapeutic relationships with patients and caregivers, including providing emotional support and education.

Providers see an average of six (6) patients per day. Based on patient need, more visits may be scheduled with provider approval and are reimbursed additional to the hourly rate.

The practice uses AthenaCare EHR.



REQUIREMENTS:
- North Carolina NP or PA license
- DEA
- 2+ years advanced practice experience preferred
- experience in home health, acute care, and palliative care preferred
- new grads and less experienced providers considered on an individual basis
