Seeking an experienced Nurse Practitioner or Physician Assistant to join a post-acute care program in Oxford, New York. This part-time opportunity offers a consistent two-day per week schedule within an inpatient post-acute care setting, focused on comprehensive primary care and interdisciplinary collaboration.
Position Highlights:
Part-time schedule, two days per week (Tuesdays and Thursdays)
Inpatient post-acute care setting
Deliver comprehensive primary care services
Manage acute and chronic conditions
Develop individualized patient care plans
Work closely with interdisciplinary care teams
Well-developed infrastructure with extensive back-office support
90-day onboarding program with MIPS training and support
Growth opportunities into local and national leadership roles
Qualifications:
Nurse Practitioner or Physician Assistant
FNP, CNP, AGNP, APRN, or PA certification
Active New York NP/PA license
Active DEA
Experienced clinicians preferred
Compensation & Benefits:
Productivity-based compensation model (fee for service)
Estimated annual compensation range of $49,096 – $58,917
