Responsibilities:

• Collaborate with physicians and healthcare team members to provide continuous outpatient care.
• Perform comprehensive health assessments, including physical exams and psycho-social evaluations.

• Prescribe medications and non-pharmacologic therapies, monitor treatment efficacy, and modify care plans accordingly.
• Participate in hospital rounds, assist with patient discharges, and provide patient education.
• Document all clinical activities accurately in the EMR system.
• Engage in quality assurance activities and support performance improvement initiatives.
• Refer patients to specialists or community resources as needed, ensuring holistic care delivery.

Requirements:

• Graduate of an accredited Physician Assistant Program and licensed in New York State.
• 2–5 years of related clinical experience preferred; new graduates with strong training and clinical skills are considered.
• Demonstrated proficiency in health assessment, clinical decision-making, and patient management.
• Strong communication skills, flexibility, organization, and the ability to work under pressure.
• Commitment to maintaining patient confidentiality and professionalism in all interactions.
• Active participation in continuing education and quality improvement activities.
