Responsibilities:
- Collaborate with physicians and healthcare team members to provide continuous outpatient care.
- Perform comprehensive health assessments, including physical exams and psycho-social evaluations.
- Develop differential diagnoses, order diagnostic tests, and interpret results.
- Prescribe medications and non-pharmacologic therapies, monitor treatment efficacy, and modify care plans accordingly.
- Participate in hospital rounds, assist with patient discharges, and provide patient education.
- Document all clinical activities accurately in the EMR system.
- Engage in quality assurance activities and support performance improvement initiatives.
- Refer patients to specialists or community resources as needed, ensuring holistic care delivery.
Requirements:
- Graduate of an accredited Physician Assistant Program and licensed in New York State.
- 2–5 years of related clinical experience preferred; new graduates with strong training and clinical skills are considered.
- Demonstrated proficiency in health assessment, clinical decision-making, and patient management.
- Strong communication skills, flexibility, organization, and the ability to work under pressure.
- Commitment to maintaining patient confidentiality and professionalism in all interactions.
- Active participation in continuing education and quality improvement activities.
