COMPANY PROFILE:
This organization is a Medicare Advantage Institutional Special Needs Health Plan (ISNP) for long-term nursing home residents. The plan covers all medically necessary and preventive services covered under Medicare Parts A, B, and D. They are strongly focused on improving their members' physical, mental, and social well-being through personalized clinical care and therapies. The organization operates an integrated health plan care management platform designed to improve clinical outcomes and reduce unnecessary medical interventions for long-term care beneficiaries in skilled nursing facilities (SNFs). They employ advanced practice providers supported by nurses and other clinical team members to work with facility staff, physicians, and caregivers to manage member care in place. They are currently hiring due to growth.


POSITION DESCRIPTION:
The APP Clinical Manager reports to the Medical Director of Clinical Services. The role is responsible for overseeing the day-to-day activities conducted by the clinical delivery providers working in the health plan’s affiliated SNFs. This includes managing local facility relationships, engagement of SNFs in the health plan’s model of care, training and monitoring assigned APPs, and overall team performance.

The core duties of the role include, but are not limited to:

- conduct rounds at SNFs on a regular basis to determine if APPs are meeting standards
- conduct periodic review meetings with SNFs to review clinical and quality results
- meet with SNF leadership to discuss successes and opportunities for improvement
- review daily clinical performance metrics and take action if not on target
- provide coaching and coverage for APPs as needed
- conduct hospitalization reviews
- facilitate APP onboarding and hiring, including conducting facility and clinical orientation
- build effective SNF relationships to foster program success in the market
- work collaboratively with PCPs and other physicians
- ensure member care coordination is well-managed including transitions to community care
- identify learning needs of the team and work with other leaders to facilitate
- assist with SNF readiness assessments
- complete HEDIS / Stars measure reports with APPs and initiate actions when needed
- maintain weekly clinical update calls to facilitate interaction and information sharing
- perform chart audits as per guidelines and provide feedback if interventions are needed
- have 1:1 meetings with APPs to review scorecards and provide input on performance
- provide coaching and mentorship to the team
- provide APP coverage for facility onboarding and PTO / sick time as needed
- provide oversight / coordination of prior authorizations
- facilitate MTM compliance and communication
- initiate, track, and update APP employment requirements

Overall, the Clinical Manager is responsible for overseeing, scheduling, and supporting a team of APPs to ensure clinical quality and affordability initiatives are executed effectively and targeted outcomes are achieved.

The Clinical Manager will travel within the territory of Dayton and Cincinnati, based upon the needs of the SNFs and APPs in the market.


REQUIREMENTS:
- Ohio NP or PA license
- DEA
- 3+ years advanced clinical practice experience
- 1+ year experience with SNFs, LTC facilities, etc.
- 1+ year clinical leadership experience
