Practice Highlights:

- Work alongside pharmacists on medication management
- Complete 60-day wellness visits
- Acute and follow up visits as necessary
- Implement an individualized plan of care for patients to achieve their goals
- Assist with transitional/discharge planning and placement recommendations
- Clinically support active older adults through the aging process
