**Internal Medicine APP Opportunity in Oregon** _x000D_

-Full-time; 40 hours per week, includes 24 patient contact hours._x000D_
-Provide urgent, same-day or next-day access._x000D_
-Provide interim co-management of chronic illnesses (titrating medications, interval routine follow-up care)._x000D_
-Conduct CMS Annual Wellness Visits as needed._x000D_
-Ensure continuity of care during transitions (retirements, medical leave, vacations, etc.)_x000D_
