Job Description:

The Practice: Mayfair Internal Medicine has been at our current location for 25 years and our 2 physicians and nurse practitioner have over 50 years of combined patient care experience. Our physicians are board certified in Internal Medicine and our nurse practitioner is board certified as an Adult-Gerontology Acute Care Nurse Practitioner.

Our practice is a member of the independent practice association (IPA), PHPprime. PHPprime provides wrap-around services to help make our practice successful, including care coordination, practice transformation coaching, risk adjustment coding education, population health management, data and analytics, specialist network management, and large group contracting.

The Position:

  • Performs a comprehensive patient health assessment, including health history, physical exams and preventive screening.
  • Orders, administers and analyzes diagnostic tests including onsite X-ray, EKG, phlebotomy and urinalysis.
  • Performs advanced technical procedures.
  • Establishes systematic follow-up for evaluation of the plan of care and reassesses and modifies the plan, as necessary, including initiation of referrals and consultation with physicians and other health care providers as appropriate.
  • Maintains accurate, timely, and confidential records.
  • Trains patients and families on managing illnesses or injuries and disease prevention.
  • Develops and maintains a good working relationship with the healthcare team.

Qualifications (Education/Experience/Knowledge/Skills/Abilities):

  • Possess current, valid licensure as an Advanced Practice Provider from the state of Colorado
  • Valid Prescriptive Authority in the State of Colorado
  • Current Drug Enforcement Administration (DEA) certification

Workdays: Monday - Friday from 9 AM - 5:30 PM
