About the Role
You will play a vital role in delivering compassionate and effective care to our patients in post-acute care communities. Your responsibilities include:
- Providing comprehensive primary care services to a manageable patient panel of 15-20 patients per day
- Collaborating with interdisciplinary teams to optimize patient outcomes
- Ensuring smooth transitions of care with hospice, palliative care, and home care agencies
- Maintaining accurate and timely documentation in our EMR system (Gehrimed)

Qualifications
- Graduate degree as a nurse practitioner or physician assistant
- Active and unrestricted NP or PA license in North Carolina
- Strong clinical assessment and diagnostic skills
- Excellent communication and interpersonal abilities
- Experience in post-acute care preferred
