Our advanced practice clinicians (APC) are responsible for driving high quality, safe and cost effective care of patients in our local post-acute care communities. You will work on an interdisciplinary team rounding on all patients as medically necessary as required by CMS. This includes but is not limited to H&P, admission, progress, transfer, discharge, and acute/follow up. You will also work with the facility to transfer patients to the level and site of care most appropriate for their medical needs upon discharge including returning to home, independent living centers, assisted living, and/or memory care. Our APCs work closely with hospice, palliative care, and home care agencies to ensure a safe transition home. Our APCs are also responsible for timely documentation and billing through our EMR (Gehrimed) that meets Medicare guidelines.

As an advanced practice clinician, you will be part of an integrated care team providing primary care services at post-acute care facilities including skilled nursing, assisted living and other long-term care facilities. Our patient population includes long term care, short term rehab, memory care, and clinically complex patients and residents. Being action oriented and having a "can do" attitude with a high standard of quality and safety in care is a top priority to help build a strong reputation in our community to support our patients.

TeamHealth is the largest integrated (emergency medicine, hospital medicine, and post-acute care) practice in the USA and will help you develop a collaborative team based approach to patient care alongside our post-acute care behavioral health team.

You will have a scheduled presence model that allows you the flexibility to work with the CPM to set your schedule based on encounter volume and facility locations so you can find your unique balance between lifestyle and work.

Productivity based compensation model to pair with the schedule flexibility giving you great independence and control over patient care volume and your income. There is no better cross section of income and lifestyle than this program. There is no call requirement for this position.

If you have experience in internal medicine (IM), family medicine (FP), or geriatrics or you have worked in a practice setting in post-acute care, traditional medicine (primary care), outpatient, urgent care, house APC or hospitalist this could be a great fit for you.

Competitive Compensation, with an estimated base salary range of $105,000 to $120,000 annually with the opportunity to earn incentives.
