Are you an experienced post-acute care nurse practitioner (NP) who aims to make a difference in the lives of your patients by providing quality care? TeamHealth needs someone like you! We are seeking a part-time nurse practitioner (NP) to work in a skilled nursing facility environment rounding on patients 3 to 4 day a week. Key Responsibilities:
  • Perform primary care in a skilled nursing facility setting for short term and long-term patients
  • Manage chronic disease, diagnose illness and medical conditions, and implement plan of care
  • Order and interpret lab results
  • Prescribe and monitor medications
  • Consult with physicians and coordinate patient care with other healthcare providers
  • Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
  • Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
  • Promote healthy lifestyle changes

Requirements:
  • 2+ years of experience in a post-acute care setting required
  • Valid NP license in the state of New Jersey
  • Valid DEA w/prescriptive authority in the state of New Jersey
